Birth Certificate – Required Documents

✅ For Hospital Birth (Most Common)

  • Hospital Birth Report / Discharge Summary
  • Parents’ ID proof (Aadhaar / Passport / Voter ID)
  • Parents’ address proof
  • Marriage certificate (optional but useful)
  • Child name declaration form (if name not given at hospital)

✅ For Home Birth

  • Affidavit from parents (notarized)
  • Certificate from local authority (Ward Member / Panchayat / Health Worker)
  • Parents’ ID & address proof
  • Informant declaration (person reporting birth)

✅ For Delayed Registration

(After 21 days of birth)

  • All above documents
  • Late registration affidavit
  • Permission from Registrar / Tahsildar

(After 1 year)

  • Court order (from Magistrate)

Step-by-Step Application Process

Method 1: Through Hospital (Automatic Registration)

  1. Hospital records birth details
  2. Data sent to Municipality/Panchayat
  3. Parents verify details
  4. Birth registered within 21 days
  5. Download certificate online or collect copy

Method 2: Online Application

Use:

  • Kerala Sevana Civil Registration Portal
  • Civil Registration System (CRS) India

Steps:

  1. Visit official portal
  2. Create login / sign in
  3. Select “Birth Registration”
  4. Enter details:
    • Child name, DOB, place
    • Parent details
  5. Upload documents
  6. Submit application
  7. Note registration number
  8. Track status online
  9. Download certificate after approval

Method 3: Offline (Municipality / Panchayat Office)

Steps:

  1. Visit local body office
    (e.g., Thiruvananthapuram Municipal Corporation)
  2. Collect birth registration form
  3. Fill details carefully
  4. Attach required documents
  5. Submit to Registrar of Births & Deaths
  6. Receive acknowledgement slip
  7. Verification by authority
  8. Collect certificate after approval

Time Limits & Rules

  • Within 21 days → Normal registration (no fee or minimal fee)
  • 21–30 days → Late fee + permission
  • 1 month–1 year → Written permission required
  • After 1 year → Court order mandatory
